Here at United Indian Health Services, we’ve always believed that good healthcare isn’t just about being there when you’re sick. It’s about helping you stay well. That’s why our team has been working behind the scenes on something called population health and we wanted to tell you a little about what that means and why it matters.
What Is Population Health?
Population health is a way of looking at the health of our entire community, not just one client at a time. Instead of waiting for people to come through our doors when something is wrong, we look at the bigger picture. Are our clients up to date on their screenings? Are there people in our community who haven’t been seen in a while and might need support? Are there gaps in preventive care that we can help close?
It’s healthcare that comes to you, rather than waiting for you to come to it.
Why Does This Matter?
Many health conditions like diabetes, high blood pressure, and certain cancers can be prevented or caught early when they’re most treatable. A screening that takes just a few minutes could make all the difference. But life gets busy. Transportation can be hard. Appointments slip through the cracks. We understand that, and we don’t want those everyday barriers to stand between you and your health.
We also know that health disparities are real and cause differences in health outcomes that affect some communities more than others. We are committed to doing our part to close those gaps. Population health is one of the most powerful tools we have to do exactly that.
What We’re Doing
Our small but dedicated population health team Rebekah Becker, our Quality Improvement Manager; Julia Metro, QI Specialist; and Dr. Joanna Hooper, our Medical Director reviews health reports to identify where our community might benefit from outreach. We look at things like which clients are due for cancer screenings, diabetes check-ups, immunizations, and other preventive services.
But here’s the important part: behind every report is a real person, and we never lose sight of that. The data simply helps us know who might need a helping hand.

If you receive a phone call from Julia or from your care team, know that it’s because we care about you. These calls are simply a friendly reminder that you may be due for a visit, a screening, or a check-in. There’s no judgment just an invitation to take a step that could make a real difference in your health.
Think of it as your health team looking out for you, even when you’re not in the clinic.
Our Goal
We want every person in our community to have the opportunity to catch health problems early when they’re easier to treat and sometimes even curable. We want to make sure that no one falls through the cracks simply because life gets in the way. And we want you to know that when we reach out, it comes from a genuine place of caring.
